New Clients

***Insurance card and photo ID are required in order to complete registration. Completing registration without submitting your documents will result in a delay in scheduling***

New clients may complete the registration online by following ALL THREE of the steps below:

Step #1

We verify your insurance benefits to obtain coverage information including copays and deductibles. Upload your Drivers License and health insurance card below.

Step #2

Complete the patient registration documentation by registering for an account on the Theranest client portal utilizing the link below. You will be prompted to create a log in, read over the policies, complete the medical history, e-sign and submit.

Step #3

For telehealth appointments, download the VSee app used to complete video counseling appointments. This app may be downloaded on your phone, tablet, laptop, or desktop.
